Sorry, the host was down over the weekend.
I just placed 7.6 MB in the usual place.  I FTP'ed ICASE's copy over.
I will send over UCSC's copy shortly.  I'll update LLNL's copy and
UCB's copy if someone gives me a shout.  Restrict copying to the
Pacific Standard Time evening or weekend.

Send additions, corrections, etc. to me.

Kudo's to Bill Nitzberg who was able to finish some editing for me.

I didn't get anywhere near the additions I wanted to get.
Many many new projects have references inside, I tried for an across the
board sample rather than a sequential conversion.
